Alex’s birthday happens to be the same date and time as their plane departure. Perhaps that’s why he has these premonitions where he can see the future – or Death’s moves.
Their Ending Spoiler
He manages to survive the movie. Death skips him and gets Carter. I think the idea was to bring him back for the second installment, but IRL, Sawa was doing too many drugs, so filmmakers kill him off between movies.
